The use of dental implants to replace missing teeth is becoming increasingly common. They may be used to replace a single tooth or multiple teeth and they can be used to support dentures in patients who find denture wearing difficult.
At the consultation, your dentist will assess the general health of your teeth and gums, as dental implants should only be placed in a healthy dentition. X-rays are used to help determine that there is enough bone to accommodate the implant and a scan may also be recommended to provide more information on the available bone volume. Sometimes bone grafting procedures are needed to augment the existing bone. All the available options for replacing the missing teeth are considered and discussed and if implant placement is the best option, you will be given a quotation for the treatment. Your dentist is kept fully informed of the planned treatment.
Placing a dental implant involves a minor surgical procedure, which is carried out under local anaesthesia. Sedation is available on request. The implant is usually left in the bone for three months, over which time the bone grows over the implant surface, a process known as osseointegration. The implant may be buried beneath the gum, in which case a minor surgical procedure is required prior to the impressions, to expose the head of the implant, or the implant may project through the gum as a small stud.
After osteointegration, an impression of the dental implant head is taken and of the opposing jaw, to manufacture the crown or bridge. In complex cases this may takes several appointments, to ensure that the best possible result is achieved. The crown is then attachment to the implant so that it emerges from gum, much the same as a natural tooth.
Implants placed in healthy mouths, with good bone levels and in non-smokers have a high success rate, with greater than 90% survival over 10 year’s. Smokers should consider quitting smoking before embarking on implant treatment, in order to have the best possible chance of success.
Implants need to be placed in sufficient bone, and this becomes more difficult at the back of the mouth; there are air sinuses in the upper jaw which reduce the available bone volume and in the lower jaw a nerve canal. The available bone is assessed with a 3D scan, which allows better visualisation of the bone and from which measurements can be made for implant planning. Bone grafting may be used to augment the bone and this is discussed on an individual basis.
Like natural teeth, implants need to be cleaned meticulously and patients are advised to see a hygienist regularly, as it is possible that gum disease can develop around implants. You will be asked to return for regular reviews to check the health of your dental implants.
